Admission Procedure
St. Mary’s welcomes admissions from the hospital, from home and respite care. We accept Medicare, Medicaid and most secondary insurances.
Hospital Admission Procedure
St Mary's works diligently with all hospitals and their discharge planners to ensure that your loved one is admitted effortlessly.
Admission Criteria Is as follows:
- A 3-day hospital stay is required in order for Medicare to cover your loved one's stay.
- Insurance verification is completed prior to admission. Insurances Include:
- Medicare
- Medicaid
- Blue Cross
- And any other secondary Insurance
- Medical paperwork from the hospital is needed in order to assure that your loved one receives the appropriate care that they require. An example of paperwork may include:
- Current chest x-ray
- History and physical
- Medication list
- Progress notes
- Therapy notes
- Once the paperwork is approved and there is a bed available, an admission date and time is scheduled with the discharge planner and family member.
Home Admission Procedure
St, Mary's welcomes admissions from home. Home admissions are generally a long-term solution when a family has tried other alternatives. We understand and the emotional stress and anxiety that you may feel and we will assist you in making the transition a very smooth one.
Admission Criteria is as follows:
- An application for admission must be completed
- Items needed from the primary care physician include:
- Chest x-ray
- History and physical
- Current medication list
- Preadmission screening forms and a home admission form provided by St. Mary's
- Once the medical information is received, a 30-minute assessment is scheduled with the Director of Nursing. Once the assessment is complete, the family will be informed of an approval or denial
- If the admission is approved, an admission date and time will be scheduled with the family
- Items needed on day of admission will include:
- If private pay, a one-month fee is collected
- If Medicaid, the patient pay amount is collected
- Insurance cards and social security card is needed to make copies
- Guardianship or power of attorney papers are needed
- 7-10 days worth of personal clothing is needed
Respite Care is for those families who may just need a break from the day-to-day stresses of taking care of their loved one. We provide 24-hour care for as little as 4 days to 10 days, Respite care works similar to a home admission, but is private pay only. Information on respite care is available through the Admission Department.
